Room Reservation Request Form
NOTE: This form is NOT to request a room reservation. You must submit requests to the department managing the specific room being requested.
This form IS intended to identify the responsible parties hosting an external organization on our campus; and to provide the institutional expectations of this process.
General Information
The information below is intended for staff responsible for processing departmental/institutional room requests at Icahn School of Medicine at Mount Sinai.
The new room reservation request form linked below may be used if there is not an existing room request form.
However, the new room reservation request form does not need to replace existing room request forms that may include room-specific information.
Importantly, all existing room request forms must be updated to collect all of the information and incorporate all of the institutional policies contained in the new room reservation request form.
Guidance for Meetings/Events with External Organizations
Please provide the name of the external organization. The organization will be required to submit a Certificate of Liability Insurance when the room request is confirmed. Click HERE for the insurance requirements.
The Meeting Host/Sponsor is responsible for reviewing the content and language on all promotional materials to ensure they are compliant with institutional policies.
All promotional and meeting materials must comply with institutional policies, including the Employee Rules of Conduct (use your Mount Sinai credentials to login) and/or the Student Code of Conduct which prohibit all forms of discrimination and hate speech.
For meetings/events with an external organization, the Meeting Host/Sponsor will receive an email confirming their accountability for policy adherence.
